One of the most distressing reports in patient feedback is a dose that looks delivered — the dial advanced and returned to zero — but no fluid appeared. 1. The Cartridge
Empty or nearly empty: when little medication remains, the final volume can be retained in the dead space of the pen and never reach the needle tip. Checking the remaining volume before injection prevents this.
Air pocket: a large air pocket in the headspace can pass the needle tip before liquid does. Re-priming, where the product procedure allows it, moves it back.
2. The Technique
The needle was not attached firmly, so the medication leaked around the hub instead of through it.
The needle was not penetrating the skin at all — for example, pressed against clothing or a bandage.
The hold time was not completed, so flow was interrupted at the moment the pen was lifted.
3. The Mechanism
A stuck drive: the dose dial turns but the internal plunger does not advance. This is a mechanical fault and cannot be repaired by the patient.
A degraded gasket or seal inside the cartridge, which lets fluid bypass the needle.
The Correct Response
Do not re-inject. Review the checklist above in order. If technique and cartridge are both correct and the mechanism still does not deliver, stop using the pen, keep it, and report the event with a description of what happened — date, dose dialed, and observed behavior. What Reuse Does to the Needle

Temperature and Handling

Concentrated Formulations
Higher-concentration preparations (for example U-200 or U-400) deliver the same amount of active substance in a smaller volume, which is why their pens use smaller cartridges and finer dose increments. The unit-to-volume relationship differs, and the dose window is calibrated accordingly — a patient switching between concentrations must follow the specific product's labeling, not translate doses between them.
What a Supplier Guarantees
For the finished product, dose accuracy is a tested requirement: the delivered volume must stay within a defined tolerance of the selected dose across the full range, across the storage temperature envelope, and across the needle-combination matrix.
